National Ability Service values and respects the privacy, confidentiality and dignity of our Participants and their families, as well as our staff. We collect, use, protect and release personal information in full compliance with all relevant State and Federal privacy legislation.

YOUR PERSONAL INFORMATION

National Ability Service will only collect information necessary for safe and effective service delivery. We will only use the information for the purpose it was collected and secure it safely. 

When we collect your information, we will explain why we are collecting the information and how we plan to use it. We will only take photos or videos of you with your full and voluntary consent. 

Your information will never be disclosed to any third parties unless you provide consent for us to do this.  There may be situations when we are required to share information such as, in an emergency, or where such disclosure is required or authorised by law. 

If you have consented to give National Ability Service access to your NDIS plan, at any time, you can withdraw your consent and request that your plan be removed.

ACCESS TO YOUR INFORMATION

You are entitled to correct, update, delete or request access to your information. For this action to take place, please email our office manager at [email protected]

NATIONAL ABILITY SERVICE OBLIGATIONS

Any personal information provided to us is protected under the Privacy Act 1988.  Our Privacy Policy framework is consistent with the Australia Privacy Principles.  It is also in accordance with the NDIA Privacy Policy – this document can be located at http://www.ndis.gov.au/privacy.  As a NDIA registered provider of Supports, National Ability Service, is required to comply with the NDIA Privacy Policy. 

On cessation of service, your information will continue to be stored for the required timeframe of 7 years in accordance with – ‘General retention and disposal authorities.’ 

COLLECTION OF YOUR INFORMATION

We collect information directly from you wherever possible. If this is not possible, or in an emergency, we may also need to collect information from a family member, friend, carer, or other person who can help us to provide you with appropriate care.

SECURITY OF INFORMATION COLLECTED

Your information may be held in a variety of ways. Most commonly, your information may be held as a paper record, and/ or an electronic record. National Ability Service uses a secure, password protected database to store your electronic records. Any paper records of your information is stored in a secure location.
